Our Mission

At Oak Medical – Onsite Physician Services we are creating consistent access to high quality care in the most efficient manner for patients, family members, and health care professionals in specialty hospitals, skilled nursing facilities and senior living communities. 

Elevating our care model through provider engagement, effective communication, collaboration across the care continuum, continued innovation, and care team integration. 

Our internal medicine physicians, advanced practice and specialty providers deliver onsite services with a care team focused on timely, dignified, and compassionate care delivery.

Rooted in Care. Built Around a Better Way to Serve Seniors

Oak Medical’s story began in 2014, with roots in long-term acute care and rehabilitation settings, including Brookfield Rehabilitation and Specialty Care (now Aria of Brookfield). Through these early experiences, Oak Medical’s founders began caring for patients beyond the hospital walls and recognized an opportunity to create a more connected, proactive approach to senior care.

Founded by Dr. Sidhu and Dr. Hostetler, both of whom began their careers as hospitalists, Oak Medical was built from firsthand experience. While caring for patients transitioning from the hospital into skilled nursing and post-acute settings, they saw the challenges residents, families, and care teams faced when trying to maintain continuity, communication, and timely access to providers.

They believed seniors deserved more than episodic care after a crisis. They deserved a dedicated medical partner who understood their needs, their environment, and the importance of proactive, relationship-based care.

That belief became the foundation of Oak Medical.

Oak Medical formally became Oak Medical SC in 2017, creating a physician-led organization dedicated to improving the way medical care is delivered in senior living and post-acute settings.

From the beginning, Oak Medical focused on building a care model centered around:

  • Consistent, accessible medical care where patients live
  • Strong partnerships with facility teams
  • Proactive management to help prevent unnecessary hospitalizations
  • Treating residents in place whenever appropriate
  • Supporting advance care planning and honoring patient wishes

As Oak Medical grew, so did its ability to serve communities across Wisconsin. The organization expanded beyond its initial post-acute foundation to partner with skilled nursing facilities, assisted living communities, independent living communities, rehabilitation hospitals, and memory care settings.

Over time, Oak Medical evolved from its physician-founded roots into a comprehensive advanced practice care model. The team expanded to include nurse practitioners and physician assistants who deliver collaborative, relationship-based care across multiple senior care environments.

As the needs of residents and facility partners evolved, so did Oak Medical’s services. The organization expanded clinical offerings to support the whole person, including geriatric-focused care, psychiatric services, wound care management, telemedicine capabilities, and continued advancements in proactive care delivery.

Today, Oak Medical partners with senior living communities, post-acute organizations, and healthcare facilities throughout Wisconsin to provide accessible, personalized medical care.

Through integrated electronic medical records, real-time communication tools, and participation in value-based care initiatives, Oak Medical continues to strengthen collaboration between providers, residents, families, and care teams.

Nearly a decade later, our foundation remains the same:

To close gaps in care, improve outcomes, and ensure seniors receive the right care in the right setting.

The next chapter of Oak Medical is still being written.
