Your Healthcare Team

Oak Medical is made up of a team of healthcare professionals working together to support your health. Your care may include a Nurse Practitioner (NP), Physician Assistant (PA), or Physician (MD/DO) who provides your primary medical care, with physician oversight and support from our nursing team to help ensure your needs are addressed and your care is coordinated.

What to Expect on Appointment Day

Your appointment time is approximate.

Because we provide care throughout your community, your provider may arrive a little earlier or later than your estimated time.
If another resident needs extra time, or if someone is unavailable when the provider arrives, they’ll simply see another resident first and return before leaving the building.
This flexible schedule allows every resident to receive the time and attention they deserve.

Preparing for Your Visit

To help make your visit as productive as possible, please have your current medications or an up-to-date medication list available.

Who Should I Contact?

Your community staff knows you well because they’re with you every day. Many questions or concerns can be addressed quickly by your community staff, who will communicate directly with Oak Medical whenever appropriate.

Your Patient Portal

The Patient Portal is a secure online tool that helps you stay connected to your healthcare. It’s available to you and, if you choose, your family members.


What you can do in the Portal:

  • View visit notes and care updates
  • Send secure messages to your care team
  • Check your upcoming appointment schedule
  • View medications and health information

You must receive an email or text invitation from Oak Medical before you can create your account. If you haven’t received an invitation and would like access, please contact Oak Medical.

The Patient Portal is optional. If you prefer not to use it, your care team will still communicate with you and your community as they always do.

What are on-site physician services?

Our on-site provider visits your community regularly to provide routine check-ups, address medical concerns, and coordinate care directly with your community’s nursing staff. Think of it as bringing the doctor’s office to your door.

What are the benefits of on-site physician services?

No travel or waiting rooms, more frequent check-ins, earlier detection of health issues, easier care coordination with your community’s team, and support for medication management.

I’m a long-term resident. Can I keep my current doctor?

If you choose Oak Medical, our provider becomes your primary care provider (PCP).

Will I still see my specialists if needed?

Yes. If you need a cardiologist, dermatologist, or other specialist, your Oak Medical provider will refer you just as any primary care provider would, and can help manage appointments and follow-ups.

Is this covered by insurance?

Yes. Oak Medical’s on-site physician services are covered by Medicare, Managed Care, Medicaid, and commercial payers. Deductibles and co-payments apply the same way as a typical office visit. Questions about your specific coverage? Call 262-875-5070.

Will my prescriptions be filled by this provider?

Yes. Oak Medical providers fill and refill medically necessary prescriptions through your community’s pharmacy.

Does Oak Medical have a physical clinic I can visit?

No. Oak Medical is a mobile care practice. We come directly to your community.

Can family members be involved in my care?

Yes, we encourage family involvement. Our providers and care team work closely with patients and their families to ensure clear communication and comprehensive care planning.
