New Facility – Oakwood Village

Oak Medical continues its growth in the Madison area with our newest Facility Partnership, Oakwood Village.  Oak Medical will be providing services at both the Prairie Ridge and University Woods locations.   Oakwood Village provides care to residents across the continuum.  Independent living, Assisted Living, Memory Care,  Skilled Nursing Care and Rehabilitation can be found at both locations.  Our Medical Director Dr. Sarfraz Sidhu and Physician Assistant Paige Smithback will follow patients at the facility. Oak Medical is excited to work with a facility that holds individuality, physical and emotional well-being, and independence as top priorities for their patients.

Oak Medical is excited to announce the expansion of our Wound Care and Psychiatric services within our existing partnerships.  Along with Oak Medical’s internal medicine services, Fort Atkinson Health Care Center, Ignite Medical Resort, Allis Care Center, Virginia Highlands, Holton Manor and the Rehab Hospital of Wisconsin welcomed additional services from our Wound Care Certified Providers and our Mental Health Nurse Practitioners. Oak Medical values the trust facilities have found with our providers that allow us to bring more specialized care to their residents, helping to provide comprehensive medical care within the facilities.

Oak Medical is proud to welcome our newest partnership with Green Bay Health Services as Respiratory Program Director.   Dr. Keshni Ramnanan will serve as Respiratory Medical Director and provide medical management in partnership with Laura Sweet, Nurse Practitioner.  Green Bay Health Services goal is to create an environment that affords their residents the opportunity to be a partner in the care they receive, to develop trusting relationships with their caregivers, and to help them reach the highest level of independence.  Oak Medical looks forward to helping these residents reach their highest goals.
